OK, so I'm confused....
How do PCs hit dice work? A Wizard is 8+1 hit dice (simple enough), Wiz12 is 8+2 (+1 hit point for advancing in level), Wiz13 is 8+3.... all fine and dandy until Wiz15 when he goes to 9+1 hit dice. Huh? Does this mean he rolls a die, adds it to his hit point total and then subtracts 3 hit points? The same question goes for fighters and clerics also....
What am I missing here?

I've said before, this is the single largest issue that the Greyhawk Supplement cleared up. From the original rules, it wasn't clear even when hit points were rolled (upon obtaining a level, entering combat, etc.).
Going by the charts in the original rules, unless you're creating a character of that level from scratch, you pretty much have to ad hoc it. So for the newly crowned Wiz15, I'd have him roll d6 and subtract 2, receiving a minimum of 1 hp.

These tables don't really make sense (as you've probably noticed). Per Gary all plusses are supposed to be cumulative, so when a character goes from 3+1 to 4 he rolls a full die, not 1d6-1, and is thus actually at 4+1. I'd make an exception for those cases when the table shows the character going from, say, 8+1 to 9+2 -- I'd only give him 1d6+1, not 1d6+2.
Here's my 'revised' tables showing both total hit dice and hit dice gained by level, based on how I think they're supposed to work:

Gary has mentioned that in his recent OD&D campaign he simplified this table, allowing clerics and magic-users 1d6/level and fighters 1d6+1/level, which is certainly easier, but seems to shortchange clerics vis a vis magic-users -- perhaps if the latter were given either 1d3/level or 1d6 on the odd levels but only +1 on the even levels.
